New Patient Registration
The doctors welcome new patients who live within our practice boundary area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. In addition, all new patients are required to produce photographic identification and proof of their present address.

Driving Licence
Passport
Birth Certificate
Marriage Certificate
Medical Card
Local Authority Rent Card
Paid Utility Bill
Bank/Building Society Statement
National Insurance Number Card
Payslip
Letter from Benefits Agency/Benefits Book/Signing on Card
Papers from the Home Office
P45
Unacceptable Documents:
Library Card
Video Rental Card
Health Club Card
Private Rent Book
Medical treatment is available from the date of registration. It may be useful if you can provide a copy of your repeat prescription slip from your former practice. Please contact reception for further information.
Online Pre-Registration
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Submit" button to mail your form to us. Please be aware that you will still need to visit the surgery to sign a form to confirm your details are correct plus you will be required to show two forms of identification one with your present address and one with a photograph or yourself before you can be accepted and able to make an appointment to see the nurse or doctor.
Pre-registration Form
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Temporary Residents
If you are a registered patient and have a family member staying with you for less than 3 months they can be registered as a temporary resident if they require medical treatment while they are in the area. Please ask at reception.
