Are you providing help and support to a relative, partner or friend who cannot manage because of a disability, illness or frailty?

Do we know you are a Carer? Or do you have a Carer?

Let us know by completing a Carer registration form (available at the Reception desk, or online HERE)

Hand the completed form to a member of the Reception team, and we will make a note on your medical records that you are a Carer.

The Carer Link at this surgery is Hazel.

If you would like further information about advise and support available locally, including access to respite, emotional and financial support and Carers emergency card, let us know by completing the consent on the form and we will refer you to Cheshire West Carers Support.

Free Personal Protective Equipment (PPE) for Carers

From Monday 8th March, Cheshire and Warrington Carers Trust are pleased to be able to offer free PPE for carers who don't live with the person they provide care for. Carers will be able to collect a package of gloves, aprons, face masks and hand gel from our Resource Centres in Northwich and Chester. We are aiming expand our collection points to include Winsford and Ellesmere Port during the following week.
