New Patients - Trans & Non-Binary

If you are a trans or non-binary person (used here as an umbrella term) and wish to register with the Practice, we request that you complete the registration documents with your name, title and gender which you are registered under at your previous GP Practice. This will allow us to find your records on our computer system and request them from your previous Practice. If you use the gender neutral title 'Mx' please note this on your registration documents.

If you use a first name which is different from your legal first name, please note this on your registration documents. This name will be used by staff and the automated call-in screen, and you can request to change it at any time.

You may request to change the gender which you are registered under at the Practice at any time, and do not need to have undergone any form of gender reassignment treatment in order to do so. This request should be in writing so that it can be added to your record. A member of staff will then contact you to discuss this request and the process which the Practice will follow to enable the request to go through. Please note that changing the gender which you are registered at under the Practice may affect which health screening programmes you are automatically invited for.
